We offer physical exams on a walk-in basis.
NOTE: Insurance does not cover physical exams in an Urgent Care setting.
We have a general fee of $125 for most physicals. ($150 for DOT)
We provide physical exams for the following:
- DOT
- School
- Summer Camp
- Sports
- Employment
- CHIP an many others
**Please bring your exam form so that it may be filled out at the time of visit. Also, bring any vaccine records or lab results required for your exam.**

We can complete worker's compensation cases at our clinic.
To ensure the process goes smoothly, and that all procedures are covered by the worker's compensation company, please come prepared with the following:
- Your ID or Driver's License
- A Claim Number
- Insurance Name & Mailing Address
- Adjuster Name/Phone/Fax
- Additional notes relevant to the process of submitting a claim.
If you are unsure regarding any of the above, it is important that you contact your employer to ensure proper completion of the process. This helps prevent the costs for any procedures necessary from falling onto the patient and/or their personal insurance if the claim cannot go through.
When you arrive with the above information, tell the receptionist that you are here for a worker's compensation case so that they can provide you with the correct paperwork.
